This test detects the presence of bile pigments, mainly bilirubin, in urine. Normally, bilirubin is absent in urine because unconjugated bilirubin is not water-soluble and cannot be excreted in urine. Only conjugated (direct) bilirubin appears in urine when serum levels are elevated. The method used to do this test is Fouchet’s Test. Conjugated bilirubin is precipitated by trichloroacetic acid and then oxidized to biliverdin by Ferric chloride. Presence of bile pigments (bilirubin) in urine indicates hepatocellular (Liver) Disease, Obstructive (Cholestatic) Jaundice. It helps monitor liver diseases and biliary obstruction, and also detects conjugated hyperbilirubinemia.
